The Gut-Brain Axis: What Your Microbiome Has To Do With Brain Fog and Mood
If you have ever lost a word mid-sentence, read the same paragraph three times without a thing going in, or watched your mood drop for no reason you could point at, someone has probably told you to "look at your gut". It has become one of those phrases people say with enormous confidence and almost no detail.
So here is the detail. And there is a bit of national pride in this one, because a large share of the field was built a couple of hours down the motorway. APC Microbiome Ireland at University College Cork - John Cryan, Ted Dinan, Gerard Clarke and colleagues - are among the researchers who put the microbiota-gut-brain axis on the scientific map, and they coined the word "psychobiotics" along the way. When you read anything about gut bacteria and mood, anywhere in the world, you are reading downstream of Cork.
What the gut microbiome actually is
On current estimates you are carrying somewhere in the region of forty trillion microbes, the bulk of them bacteria, and the bulk of those living in your large intestine. Hundreds of species in a typical person. They are not passengers. They ferment the fibre you cannot digest, make vitamins, train your immune system, and produce a constant stream of chemical messengers that the rest of your body reads.
The word you will see everywhere is diversity - how many different species you host, and how evenly they are represented. It is a crude measure and researchers argue about it constantly, but low diversity keeps turning up alongside a long list of conditions nobody wants, and it responds to what you eat. That last part is the useful bit.
You will also see dysbiosis, which sounds precise and is not. It means "the mix looks off compared to a healthy reference", and since nobody has agreed on what the reference is, treat it as a description rather than a diagnosis.
How the gut actually talks to the brain
Well, through four routes, roughly, and traffic runs in both directions.
The vagus nerve. A physical cable running from gut to brainstem, carrying far more signal upward than down. Cut it in animal studies and several of the behavioural effects of gut bacteria disappear, which is about as direct a demonstration as this field has managed.
The immune system. Your gut lining is a barrier. When it gets leaky, bacterial fragments - lipopolysaccharide, or LPS, is the one worth knowing - cross into the bloodstream and drive low-grade inflammation. Inflammation is the most consistent biological thread running through both depression research and cognitive decline research, and the "leaky gut, leaky brain" idea is being taken seriously in this year's reviews on treatment-resistant depression.
Microbial metabolites. When your bacteria ferment fibre they produce short-chain fatty acids - acetate, propionate and butyrate. Butyrate feeds the cells lining your colon, helps keep that barrier tight, and crosses into the brain where it influences gene expression in ways that, in animals, affect memory and synaptic plasticity. Human trial evidence is considerably thinner than the mechanistic story. That gap is the honest summary of this entire field.
Tryptophan and the stress axis. Gut microbes have a hand in how tryptophan gets used - either toward serotonin, or down the kynurenine pathway, which is the inflammatory branch. They also interact with the HPA axis, the cortisol system. Cork research found the microbiome regulates stress responsiveness on a daily rhythm, which raises the possibility that when you eat matters as well as what.
While we are here: the 95% serotonin line
You have seen it a thousand times. "95% of your serotonin is made in your gut, therefore gut health controls your mood." The first half is true. The conclusion is not.
Gut serotonin and brain serotonin are two separate pools, made by two different enzymes, and serotonin made in the gut does not cross the blood-brain barrier. Your brain makes its own. Gut serotonin is running your digestion, your gut motility and your platelets, not your mood. The gut genuinely does influence how you feel - but through the vagus nerve, the immune system and metabolites, not by posting serotonin up to head office. Anyone telling you otherwise has either not read the papers or is hoping you have not.
Where the research is pointing in 2026
Treatment-resistant depression. Roughly a third of people with major depression do not respond adequately to standard treatment. A review accepted this June in Progress in Neuro-Psychopharmacology and Biological Psychiatry sets out the case that the gut-brain axis - barrier integrity, LPS, chronic low-grade inflammation - may be part of the reason why. That is a hypothesis under test, not a settled finding, but it is where serious researchers and serious money are heading.
Tryptophan as the unifying frame. Recent work is trying to pull inflammation, stress, barrier function and neuroplasticity into a single model built around tryptophan metabolism. Whether it holds up is another question, but it is the most coherent attempt yet at joining the dots.
Personalisation. The most repeatable finding across intervention trials is that people respond differently depending on the microbiome they started with. Which is why the field is drifting away from "everyone take this" and toward sorting people first.
Coffee, of all things. APC published work in Nature Communications this April on the mechanisms behind coffee's effects on the gut-brain axis, in both caffeinated and decaf forms. Worth noting the study was sponsored by the coffee industry's own research institute, which does not invalidate it but is exactly the sort of thing you should always be told.
And the field is being straight about its own limits, which I respect. In July, Cryan and colleagues argued in Nature Mental Health that almost all of this research has been done in wealthy industrialised countries, and that we may be mistaking the microbiome of the western diet for the human microbiome.
Brain fog and cognition
Brain fog is not a diagnosis. It is a description - slow recall, poor concentration, the sensation of thinking through wet sand - and it can come from sleep debt, stress, thyroid problems, perimenopause, long covid, anaemia, medication, or simply a rotten fortnight. Please do not assume it is your gut. Get the ordinary explanations ruled out first.
That said, there is a real signal here. Cohort studies find that gut microbial diversity tracks with cognitive scores in both healthy and ageing adults. A systematic review published earlier this year in Nutrition Research pulled together trials of microbiome-targeted approaches in adults over 45 with cognitive impairment or dementia risk, and found that quite different routes - Mediterranean-style eating, probiotics, fibre - converge on the same biological pathways, with a strong hint that timing matters and earlier is better.
What I will not do is tell you a capsule clears brain fog. The gap between doses used in research and doses that appear in commercial products is real, and the people selling them rarely bring it up.
Mood and depression
This is where the evidence is strongest, and it still needs stating carefully.
A 2025 systematic review and meta-analysis in Nutrition Reviews, covering 23 randomised controlled trials and 1,401 clinically diagnosed patients, found probiotic supplementation produced a substantial reduction in depression symptoms and a more moderate reduction in anxiety, with eight weeks enough to show an effect. An umbrella review published this January - a review of the reviews, which is about as high as the evidence pyramid goes - landed on the same shape: moderate and reasonably consistent benefit for depressive symptoms, only minor effects for anxiety.
Three caveats matter more than that headline. Nearly all of this was tested alongside standard treatment rather than instead of it, so nothing here is a reason to stop taking anything you have been prescribed. Strains are not interchangeable - Lactobacillus and Bifidobacterium species dominate the trials, and results attach to specific strains rather than to "probiotics" as a category. And the trials are mostly small, mostly short and varied enough that the authors of every one of these reviews end up saying the same thing: promising, not proven.
So what actually builds healthier gut flora?
Plants, and lots of different ones
Fibre is the substrate. No fibre, no fermentation, no short-chain fatty acids. Variety appears to matter as much as volume - the thirty-different-plants-a-week idea is a rough heuristic rather than a law, but it pushes you in the right direction, and herbs, spices, nuts, seeds, pulses and wholegrains all count toward it.
One honest wrinkle. In the Stanford trial I am about to describe, ten weeks of high-fibre eating did not raise microbial diversity at all. The researchers' best explanation was that if your microbes lack the machinery to break the fibre down, adding more of it just moves it through you. Fibre still matters enormously. It is simply not the instant lever it gets sold as, and if you have been eating like a student, build it up slowly.
Fermented food
This is the most striking dietary result in the field. Stanford randomised 36 healthy adults to ten weeks of either high-fibre or high-fermented-food eating - yoghurt, kefir, kimchi, sauerkraut, kombucha. The fermented group increased microbial diversity and showed decreases across 19 inflammatory proteins in blood. The fibre group did neither. A larger citizen-science replication published last year found broadly the same pattern.
Small trial, healthy volunteers, short window. But it is cheap, it is food, and the effect sizes are not subtle. A daily portion of something fermented is the easiest change on this page.
Sleep, stress, alcohol and antibiotics
The unglamorous half. Your microbiome runs on a daily rhythm, so shift work and broken sleep disturb it - and the sleep piece is worth fixing on its own merits before you spend a cent on your gut. Chronic stress alters the microbiome through the same HPA axis it disrupts in you. Alcohol is hard on the gut barrier. Antibiotics are sometimes necessary and always costly to microbial diversity, so take them when you need them and not when you do not.
Exercise independently associates with higher diversity. Nobody in wellness wants to hear that the answer is sleep and walking, but it keeps being the answer. Meal timing belongs in this conversation too, which is a rabbit hole I have already gone down in the honest fasting guide.
And where supplements come in
Last. Genuinely last, because a supplement stacked on top of a diet of beige food is money poorly spent. If you want the broader argument for building the base first, it is the same one I made about optimum nutrition.

And if your problem is honestly digestive rather than neurological - heaviness, bloating after meals - a digestive enzyme complex is a more sensible first stop than any of the above.
Do not neglect the fats either. The bottom line
The gut-brain axis is real, the mechanisms are increasingly well described, and the mood evidence has moved from interesting to reasonably solid inside a decade. That is fast for nutrition science, and the people in Cork deserve a good share of the credit.
What has not happened is the part the internet has already announced. No test tells you which bacteria you are missing. No probiotic replaces treatment for depression. What you have instead is a set of levers that are cheap, safe and slow: eat more plants and more kinds of them, eat something fermented most days, sleep properly, move, drink less, and take antibiotics only when you genuinely need them. Supplement on top of that if you want to, with realistic expectations and eight weeks of patience rather than eight days.
